During this time, LCBC would like to launch another Bingo to help us practice self care and keep a little humor in our lives.

Rules
- We’re on the honor system here, folks. :-)
- Complete as many tasks as you can by the deadline: May 31, 2020.
- For each task, check off its box.
- You can print if you like and then scan and email the final or...
- You can just use the digital copy and click the box to check it.
- Don’t count tasks you already completed before you received this form.
- When you’re done, calculate the number of “bingos” you have, and type the number in the box provided.
- Type in your name in the box.
- Basic bingo rules: we count horizontal, vertical, and diagonal bingos only.
- When you’re done either:
- For paper copies: take a picture and email it to Tina Griffin: tmcg@uic.edu. Please put the word “Bingo” in the subject line.
- For digital copies: Upload it using the widget below.
- The top 10 people with the most “bingos” will win prizes (depending on the number of entries).
Prizes
- Dinosaur taco holder, (specify tricerotops or T-rex). Each dino holds 2 tacos.
- Beer and Food pairing guide Towel
- Mini Buddha paint with water Zen board
- Knock Knock notepads (pick two)
