Hello students and residents at the College of Dentistry, 

Through a collaborative research project, the Library of the Health Sciences and the College of Dentistry will be surveying students and residents on their Knowledge, Attitude, and Confidence in Evidence-Based Practice, also known as a KACE survey.

This survey is optional. For your review, the survey consent information is available at the QR code on the screen at the front of class, I also have physical copies as well. Physical copies of this script are also available as well if you would like one for your records. Beyond your level in Dentistry school or residency, no other identifying materials or contact information will be collected. Those who decline participation may use this as their own quiet work time. It has been decided that the survey will only be implemented in the classrooms, and you are allotted up to 30 minutes to complete it.

We have provided the survey link for you, or you can scan this QR code. If you would like a physical copy of the survey consent information, physical copies are available for your records.
