Adding Items to the Form

After going back to edit the review form that you have created, you can now add items (form questions) to your form.

Along with adding items to your form, you will:

Choose a type of response to this item
- Text box
- Checkbox
- Radio buttons
- Dropdown menu
Select what response options you want to make available (i.e. a Likert Scale)
