Personal Inventory

Create a folder-level inventory of the papers
 

Once selected for inclusion in the UIC Archives, you must complete a folder-level inventory of all items to be donated.  A completed personal inventory must accompany all deliveries of personal papers. 

The personal inventory form, including instructions, will be forwarded once your application has been approved.
